Release Form

By signing this Client Liability Release Form, I acknowledge that I have read, understood, and agreed to the terms and conditions outlined herein. I voluntarily participate in the corporate wellness services provided by Tourmaline Health and Wellness, including chair massage, table massage, and personal training sessions. I release Tourmaline Health and Wellness and its business partners and associates from any liability related to injuries or adverse reactions that may occur during or after these services.

Terms and Conditions for Voluntary Participation in Corporate Massage and Personal Training Session

1. Assumption of Risk

I, [Client's Full Name], acknowledge that I am voluntarily participating in the corporate wellness services provided by Tourmaline Health and Wellness, including chair massage, table massage, and personal training sessions. I understand and accept that there are inherent risks associated with these activities, and I voluntarily assume all such risks, including but not limited to, physical injury, discomfort, or adverse reactions to the services provided.

2. Release of Liability

In consideration of being allowed to participate in these services, I hereby release and discharge Tourmaline Health and Wellness, its employees, agents, representatives, and any affiliated parties, from any liability, claims, demands, actions, or causes of action arising out of my participation in the corporate wellness services. This release includes but is not limited to, liability for any personal injury, property damage, or other loss that may occur as a result of my participation.

3. Responsibility for Health and Well-Being

I understand that I am responsible for informing the Tourmaline Health and Wellness staff of any medical conditions, allergies, or physical limitations that may affect my ability to participate safely in chair massage and personal training sessions. I have consulted with a healthcare provider and received approval for participation if I have any medical concerns. I agree to follow all instructions and recommendations Tourmaline Health and Wellness staff provided during the sessions.

4. Personal Property

I am solely responsible for the security and safety of my personal belongings during my participation in the corporate wellness services. Tourmaline Health and Wellness shall not be held liable for any loss, theft, or damage to my personal property.

5. Photo and Video Release

I grant Tourmaline Health and Wellness the right to use photographs and/or video footage of me taken during the corporate wellness sessions for promotional and marketing purposes, without compensation.

6. Confidentiality

I understand and agree that any personal or health-related information I share with Tourmaline Health and Wellness staff will be kept confidential.

7. Governing Law

This Agreement shall be governed by and construed by the laws of Maryland, Washington D.C., and Virginia/ United States of America.

8. Acknowledgment of Understanding

I have read and fully understand the terms and conditions outlined in this document. I am signing this Client Liability Release Form voluntarily and of my own free will.
